自发热经皮给药贴片的热促渗机制及艾灸热疗启示下精油促渗剂的应用边界

Abstract

经皮给药系统因可避免胃肠道降解和肝脏首过效应、实现局部或全身持续递送,在外用制剂研究中具有重要 价值。然而,角质层高度有序的脂质屏障仍显著限制药物透皮效率。自发热经皮给药贴片通过构建局部温热微环境, 可增加角质层脂质流动性、促进药物从基质释放、提高分子扩散速率,并改善局部血流和皮肤水合状态,从而增强 药物经皮递送。艾灸作为传统外治热疗形式,其现代机制涉及温热刺激、红外辐射、局部循环调节及艾草挥发性成 分作用,为自发热贴片设计提供了“温热适度、持续稳定、重视个体差异”的启示。精油及其萜类成分具有亲脂性、 小分子和角质层脂质扰动能力,可作为天然促渗剂与热促渗形成协同。但该协同并非无边界增强,受温度、浓度、 成分稳定性、皮肤状态、贴敷时间及药物安全窗共同限制。本文围绕自发热贴片热促渗机制、艾灸热疗启示及精油 促渗剂应用边界进行综述,以期为外用自发热贴片的安全设计和转化应用提供参考。
