全身炎症指标与急性缺血性脑卒中患者颈动脉斑块不稳定性的相关性

Abstract
目的:探讨全身炎症指标 - 全身免疫炎症指数(SII)、全身炎症反应指数(SIRI)、全身炎症总指数(AISI) 与急性缺血性脑卒中(AIS)患者颈动脉斑块不稳定性的相关性。方法:回顾性纳入 2023 年 4 月至 2024 年 5 月中国 科学技术大学附属第一医院神经内科收治的符合入组标准的急性缺血性脑卒中合并颈动脉斑块患者 133 例。根据颈 动脉超声结果将患者分为稳定斑块组(47 例)、不稳定斑块组(86 例)。比较两组一般资料及全身炎症指标的差异。 采用二元 logistic 回归探讨颈动脉斑块不稳定的危险因素。受试者工作特征(ROC)曲线评价全身炎症指标预测颈动 脉不稳定斑块的价值。结果:①不稳定斑块组中性粒细胞、SII、SIRI、AISI 均高于稳定斑块组,差异有统计学意义 (P < 0.05)。②二元 Logistic 回归分析结果显示 SII 是 AIS 患者颈动脉斑块不稳定性的独立危险因素(P < 0.05)。 ③ ROC 曲线显示 SII、SIRI、AISI 以及三者联合预测颈动脉斑块不稳定的曲线下面积(AUC)分别是 0.708、0.662、 0.645、0.745,差异有统计学意义(P < 0.05)。结论:SII、SIRI、AISI 升高与 AIS 患者的颈动脉斑块不稳定性相关, 对颈动脉不稳定性斑块有一定的预测价值。
